Disciplinary Records Check Request


When filling out the Disciplinary Records Check Request Form, you will be asked to indicate how you would like the form returned. Provide either a fax number for the requesting institution or email address.

A disciplinary reference request only provides information from the Office of Student Rights and Responsibilities. If necessary, you will have to contact the Office of the Registrar separately for records regarding your academic standing.

Academic institutions and employers can also request a student/alumni’s disciplinary record directly by filling out the Disciplinary Records Check Request Form.

Please note that you will need to provide a signed release from the student/alumni for this information to be provided.

Procedure

A disciplinary records check can take up to 7 days, so be mindful of your application deadline.

Maintenance of Records

Records regarding student code of conduct violations will be maintained for a period of eight years. If you were suspended or expelled due to a code of conduct violation, your records will be kept permanently.

Records pertaining to Academic Integrity violations by graduate students will be retained by our office for eight years from time of incident. Records pertaining to Academic Integrity violations by undergraduate students will be retained by our office for eight years from time of the academic year of the offense.

The Office of Student Rights and Responsibilities does not release the following types of cases:

  • Cases in which the student is found not responsible
  • Cases where informal action or no action is taken
  • Cases where Medical Amnesty is granted

If you would like to complete a Disciplinary Records Check, you will need to review and complete the Disciplinary Records Check Request Form.

Student Conduct Records Check for Internal Purposes

Faculty and Staff members may submit a student record check using the Disciplinary Records Check Form. Please follow the instructions below to complete your request.

Please allow one week for requests with less than 100 students. Please allow at least two weeks for requests with over 100 students.

Student Conduct Records Checks take no more than ten business days to complete. What our office discloses to you includes:

  • The date of the incident
  • The policies violated (or a summary if the policies may not be completely clear)
  • The sanction assigned

We will note if there is a case pending.
